It is no longer necessary to keep a daily log. If you experience COVID-19-like symptoms, it is required to report online or by phone.
The paper version of the Daily Symptom Checker is no longer being used or necessary to track daily symptoms.
Employees may also contact their supervisors, who have the authority to fill out the online Symptom Reporter on their behalf.
Supervisors do need to keep paper forms submitted by employees prior to Oct. 26 for three months.
