When does a supervisor complete the Daily Symptom Checker on behalf of an employee?

A supervisor reports on behalf of employee

Two primary considerations determine whether a supervisor shall complete the Daily Symptom Checker on behalf of the employee:

  1. The employee does not have access to the internet away from university property and therefore cannot self-report, and
  2. And the employee has one of the following to report:  
    • is feeling ill or exhibiting symptoms which include new loss of ability to taste and smell, fever, chills, cough, shortness of breath, and other flu-like symptoms,
    • has been tested for COVID-19 and the results were positive,
    • is in the process of being tested,
    • is living with a COVID-19 positive person, or
    • if they believe they have been exposed to someone with COVID-19.
