All students and employees are expected and required to report any COVID-19 symptoms to the university immediately, as well as report exposures or positive tests from a non-CSU testing location.

  • Experiencing COVID-19 symptoms, which include fever, chills, cough, new loss of ability to taste and smell, shortness of breath, and other flu-like symptoms
  • Have been tested for COVID-19 and the results were positive
  • Are in the process of being tested
  • Are living with a COVID-19 positive person
  • If you believe you have been exposed to someone with COVID-19

Faculty

  • Faculty should not complete the Symptom Reporter on behalf of students. 
  • To report a COVID-19 concern about a student or employee, call (970)491-4600 or email the CSU Public Health Office.

Information for Visitors

  • During this time of COVID-19, we are strongly discouraging visitors to CSU campuses.
  • Visitors should not complete the Symptom Reporter. 

Information for Supervisors

Employees are no longer required to fill out the Daily Symptom Checker each day, but are required to complete the online Symptom Reporter immediately if they have symptoms that may be COVID-19 even on days they may not be reporting to work.

If an employee does not have access to the internet, they still need to report symptoms to the university immediately by calling the Public Health COVID-19 phone number at (970)491-4600.
